Jermain Defoe converted a 15th-minute penalty and Gareth Bale scored a solo goal moments before halftime to put Spurs in control and help cut the Blues' lead at the top to a single point.
Chelsea's day got even worse in the 67th minute when captain John Terry was sent off for a second yellow card that rules him out of next weekend's match at Stoke with a suspension.
Frank Lampard scored Chelsea's goal in injury time, but Tottenham moves above Manchester City into fourth place and the Champions League qualifying places.
Three days after beating Arsenal 2-1 to effectively end the Gunners' hopes of winning the title, Tottenham dominated its other main London rival at White Hart Lane.
Bale starred down the left and Roman Pavlyuchenko was a constant threat up front, while goalkeeper Heurelho Gomes preserved his team's advantage at 1-0 with a point-blank save from Lampard's goal-bound volley.
Chelsea made only three changes from the side that beat Bolton 1-0 a week ago but rarely showed glimpses of the form that brought 14 goals in its previous three Premier League matches and took it to the FA Cup final.
Tottenham could have scored more, with goalkeeper Petr Cech saving well from Pavlyuchenko, Defoe and David Bentley before Bale shot narrowly wide with eight minutes left.
After 17 years without a league victory over Chelsea, Tottenham has now won three of its last four home matches against the Blues and drew the other 4-4 less than a month after beating the same team in the 2008 League Cup final.
Spurs had already been denied one strong claim for a penalty when John Obi Mikel shoved Bale to the ground but, from the resulting corner, Terry headed Tom Huddlestone's cross against his own arm to concede a kick.
Defoe stepped up and hit the ball firmly into the bottom left corner, with Cech diving the other way.
Chelsea did have a goal ruled out, with both Joe Cole and Florent Malouda offside as Yuri Zirkhov slipped a pass through for Malouda to turn and shoot. But Tottenham continued to stretch its opponent and went 2-0 up in the 44th.
Bale toyed with fullback Paulo Ferreira and found space to smash a shot past Cech at the near post.
Chelsea coach Carlo Ancelotti reacted by withdrawing Ferreira and Cole at halftime and putting on Branislav Ivanovic and Nicolas Anelka. But with Michael Ballack already on for the injured Mikel, the second-half kickoff was delayed when Chelsea tried to change one of its substitutes because Didier Drogba was complaining of a groin injury.
Drogba returned to the field but looked less mobile than usual and Tottenham continued to create the best chances, particularly after Terry got two yellow cards for clumsy challenges within three minutes.
Every player except Cech was involved in pushing and shoving on halfway following Huddlestone's kick on an opponent after he had been pushed over. But Tottenham remained settled, and Bale and Pavlyuchenko went close in the last few minutes before Lampard reduced the deficit from close range.
