The semiofficial Fars news agency quotes the head of the provincial Red Crescent Society, Mohammad Bagher Mohammadi, as saying two people also were injured in Tuesday's blast in the city of Ilam.
The report does not elaborate, but state TV says it was apparently a gas explosion.
The blast comes just over a week after assailants elsewhere in Ilam blasted open a prison wall with rocket-propelled grenades, allowing two convicted murderers to escape.
Ilam, some 700 km southwest of Tehran, is usually a calm area, despite containing a Kurdish majority and bordering Iraq.
