For small establishments three visas are allowed when the business is started. This could be just one visa for a start-up restaurant or workshop. The number is often decided at the local Labor Office according to the needs of the establishment. Larger companies may have larger visa limits.
Since you are here, you just need to visit the Indemnities Department at the GOSI premises. They will provide you with a form called entitlements form for non-Saudis. Fill in this form with your insurance number, full name in Arabic, old iqama, the address that you would like to receive your money at, your name and signature, and the date.
You are also required to enclose a photocopy of your passport pertaining to that period of service. That is all you need to do to get your money back.
After GOSI has verified your information, it will issue a check in your name and hand it over to you or transfer it to your account.
